After Gateway serves individuals 18 years of age and older who have a primary diagnosis of a developmental disability. We accept individuals who are ambulatory, semi-ambulatory (uses a walker, cane, etc.), or non-ambulatory; who may or may not require personal care services and/or assistance with activities of daily living (such as toileting, feeding, etc.); and who may or may not have conditions requiring medical treatments, observation and monitoring, or medication management. Applicants typically are reviewed on a case-by-case basis, however.
After Gateway will only accept applicants who are able to pay for the services rendered, either through a federal program, state program, or private pay. Individuals are required to attend a minimum of six hours per day, three days a week. If an individual presents a danger to other participants and is unable to be redirected, he or she will not be admitted to the program, or be allowed to continue attending the program.
To apply to the program, a completed application and at least one completed reference forms must be submitted. Upon review of the application and references, the Program Committee will determine the individual's suitability for the program, and begin the admission process.
Prospective applicants are encouraged to visit the program prior to applying. Tours are available between the hours
of 9-11 a.m. and 1-3 p.m. Monday-Friday. Individuals who wish to tour the program are encouraged to call and set
an appointment. At the time of the tour, interested parties will be given a program brochure, an application, reference forms, and specific program information including a program calendar.
To apply to the program, a completed application along with a $25 application fee, signed by the responsible party, and at least one completed reference form must be submitted. Upon review of the application and references, the Program Committee will determine the individual's suitability for the program.
In order to facilitate the transitioning process for incoming participants coming from the public school system, the following policy applies:
